Food Service would be happy to help celebrate your child's event or birthday.

To print, simply click on the form to open as a PDF. Once you have filled out the form, please return to your students school along with cash or check payment 2 weeks prior to the event. Students lunch account CAN NOT be used as a form of payment.

If you are interested in other items not listed on the form, you are welcome to reach out to the food service office. 

Parents may also want to see our Classroom Snacks page.  These items are Smart Snack approved by the State and may be brought to the school.

As always, final approval for parties and snacks will come from your student's teacher, nurse and principal. You must allow at least 2 weeks to get this approval and ensure product availability.  

PLEASE NOTE that treats will not be provided until form has been approved and payment is made.

Please contact your school location prior to ordering. Some schools may have additional restrictions or requirements on parties.
