Health Requirements

  • Immunization Requirements

    • 4 doses of tetanus (1 dose after the 4th birthday)
    • 4 doses of diphtheria (1 dose after the 4th birthday)
    • 4 doses of polio (1 dose after 4th birthday and 6 months after last dose (the 4th dose of polio must be given after the 4th birthday AND at least 6 months after 3rd dose)  
    • 2 doses of measles
    • 2 doses of mumps
    • 1 dose of rubella
    • 3 doses of hepatitis B
    • 2 doses of varicella(chickenpox), or history of the disease

    **All of these vaccines must be given BEFORE your child can attend school.  If your child is missing a dose of any of the above vaccines, you will have only 5 days to receive that vaccine or your child will NOT be permitted to attend school.

     

     

    Physical Exam Requirements

    The Pennsylvania School Code requires that students entering school from another Pennsylvania school district or another state have a physical examination if no reports have been found in the transfer records. You are encouraged to have the school medical examination done by your family physician.  If you prefer, your child may receive a physical examination by the school physician at no cost to your family.  School physicals are scheduled from December through March.

     

    Your child needs a physical examination for the following reason:

     

    • Original entry to kindergarten or 1st grade, entering 6th or 11th grade
    • Transfer records from another school district do not contain a physicians report of a physical examination
    • Student was absent day of school physical examination
    • A report of a physical exam has never been received for your child

     

    If your child is examined by your family physician, please have the examination recorded on the attached form and return the completed form to the school nurse.  If your child had a physical prior to or during Kindergarten/1st, 6th or 11th please forward a copy to the school nurse.

     

    Dental Exam Requirements

         The Pennsylvania Department of Health requires that every child entering school for the first time (kindergarten/original entry) and grades 3 and 7 have a dental examination.  Because of your dentist’s knowledge of your child’s dental history, we recommend that your dentist do your child’s exam.  

     

    You may give permission for the school doctor and/or school dental hygienist to complete the exams free of charge.
