One-time Parental Consent and Annual Parental Notice:
Parents are asked to provide written permission for the school district to bill medical assistance. Your participation is voluntary, and can be withdrawn at any time. Your child will continue to receive all services regardless of your decision to participate. Moreover, there is never a cost to the parent. Those participating need only check, sign, and date the Parent Notice and Consent forms. This consent allows the district to share a copy of the student’s IEP with a contracted physician for his/her authorization that the services recommended are appropriate.
